We offer an accessible, trauma-informed, interprofessional, and integrated Regional Lung&nbsp;Health&nbsp;Program. We work from a <a href=\"https:\/\/www.tandfonline.com\/doi\/full\/10.1080\/24745332.2019.1643610\" target=\"_blank\" rel=\"noreferrer noopener\">Respiratory Rehabilitation model&nbsp;<\/a>and the <a href=\"https:\/\/www.longwoods.com\/content\/16763\/healthcare-quarterly\/the-expanded-chronic-care-model-an-integration-of-concepts-and-strategies-from-population-health-pr\" target=\"_blank\" rel=\"noreferrer noopener\">Expanded Chronic Disease Management Model<\/a>. Some referrals might be considered for group sessions but not for individual care.
