These workshops teach skills and provide tools to better self-manage their own health and wellness.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">We also offer free workshops for&nbsp;health care professionals&nbsp;to teach them how to be better self-management supporters.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">We offer these workshops to anyone living in Southeastern Ontario.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">What is self-management?<\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Being able to manage the daily tasks needed to live well with one or more chronic conditions;&nbsp;<\/li>\n\n\n\n<li>Having the skills and confidence to take charge of your health and improve your quality of life and emotional well-being;<\/li>\n\n\n\n<li>Being as healthy as possible and achieving the things you want to do in life;<\/li>\n\n\n\n<li>Being the most important and active member of your health care team.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">What is self-management support?<\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Understanding what self-management is and why it\u2019s important;<\/li>\n\n\n\n<li>Helping others to start and continue on their own path of self-management;<\/li>\n\n\n\n<li>Client\/patient-centered; working as a team with client\/patient;<\/li>\n\n\n\n<li>Working at helping someone else self-manage their health and wellness based on where they are in terms of readiness and confidence.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">To find out more about our program, see a calendar of upcoming workshops, or learn how to become more involved, please visit\u00a0<a href=\"http:\/\/www.livingwellseontario.ca\/\">our website<\/a> or <a href=\"https:\/\/www.facebook.com\/livingwellselhinsmp\">Facebook page. <\/a><\/p>\n","protected":false},"excerpt":{"rendered":"<p>Our Regional Self-Management team offers free\u00a0community workshops\u00a0for people who live with chronic conditions, chronic pain, diabetes, and cancer, as well as for caregivers.<\/p>\n","protected":false},"featured_media":847,"template":"","categories":[16,3,13],"communities":[18,17],"class_list":["post-166","program","type-program","status-publish","has-post-thumbnail","hentry","category-diabetes","category-health-and-wellbeing","category-mental-health","communities-adults","communities-all-ages"],"acf":[],"yoast_head":"<!-- This site is optimized with the Yoast SEO plugin v28.1 -
