These services are delivered in a multi-disciplinary community-based, harm reduction setting where clients can access a number of professional including: registered treatment nurse, liver specialist, counselor, outreach worker, peers, dietician and psychiatrist.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Services offered include but are not limited to:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Hep C treatment and care<\/li>\n\n\n\n<li>Testing<\/li>\n\n\n\n<li>Advocacy<\/li>\n\n\n\n<li>Harm reduction education and supplies<\/li>\n\n\n\n<li>Naloxone training<\/li>\n\n\n\n<li>Case management<\/li>\n\n\n\n<li>One-on-one and group counselling<\/li>\n\n\n\n<li>Peer support<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Clients can self-refer directly to our team by phoning us or visiting us in person at 115<br>Barrack Street (corner of Barrack and Bagot).<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">For healthcare providers who wish to refer patients, please call us, or fax us a copy of the <a href=\"https:\/\/kchcnew.thedev.ca\/wp-content\/uploads\/2024\/01\/PDF-RAAM-REFERRAL-May-2021.pdf\">SHC Referral Form<\/a>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><a href=\"https:\/\/kchcnew.thedev.ca\/wp-content\/uploads\/2024\/01\/Hep-C-101.pdf\">More information on Hepatitis C<\/a>.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Street Health Centre\u2019s Hepatitis C program is one of 15 provincially funded teams in Ontario providing access to education, testing and treatment for individuals who have difficulty accessing mainstream healthcare services.
