{"id":1913,"date":"2024-08-07T19:54:22","date_gmt":"2024-08-07T19:54:22","guid":{"rendered":"https:\/\/kchcnew.thedev.ca\/?post_type=program&#038;p=1913"},"modified":"2024-09-05T11:08:51","modified_gmt":"2024-09-05T15:08:51","slug":"greater-napanee-health-home","status":"publish","type":"program","link":"https:\/\/kchcnew.thedev.ca\/fr\/programs\/greater-napanee-health-home\/","title":{"rendered":"Learn more about GNHH"},"content":{"rendered":"\n<p class=\"has-medium-font-size wp-block-paragraph\">The Greater Napanee Health Home (<strong>GNHH<\/strong>) launched in April 2024 &#8211; through a collaboration with the Kingston Community Health Centres, the Mighty Docs Family Health Organization, the&nbsp;Lennox &amp; Addington County General Hospital and the Town of Greater Napanee. <\/p>\n\n\n\n<p class=\"has-medium-font-size wp-block-paragraph\">Health Homes are the new model of care introduced by the&nbsp;<a href=\"https:\/\/flaoht.ca\/\" target=\"_blank\" rel=\"noreferrer noopener\">Frontenac, Lennox and Addington Ontario Health Team<\/a>, in which comprehensive ongoing primary care is provided through a team-based model.<\/p>\n\n\n\n<p class=\"has-medium-font-size wp-block-paragraph\">This GNHH provides team-based care&nbsp;at multiple sites in the Greater Napanee area. This includes providing care to more than 5,000 people in the town of Greater Napanee who do not yet have a primary care provider (family doctor or nurse practitioner) &#8211; the <strong>GNHH Access Clinic<\/strong> was developed for this purpose.<\/p>\n\n\n\n<p class=\"has-large-font-size wp-block-paragraph\">GNHH Access Clinic<\/p>\n\n\n\n<ul class=\"wp-block-list has-medium-font-size\">\n<li>For people without a primary care provider (family doctor or nurse practitioner)<\/li>\n\n\n\n<li>Appointments can be made for:\n<ul class=\"wp-block-list\">\n<li>prescription renewals<\/li>\n\n\n\n<li>acute episodic or urgent issues such as, UTIs, ear infections, eye infections, etc., (non life-threatening issues)<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Location<\/strong>: GNHH-Napanee Area Community Health Centre site<\/li>\n<\/ul>\n\n\n\n<p class=\"has-medium-font-size wp-block-paragraph\"><\/p>\n\n\n\r\n\t\t\t\t\r\n\t\t<a \r\n\t\t\t\r\n\t\t\tclass=\"block-ab9a3872-ddc3-411e-8289-f932638c45e8 btn areoi-has-url position-relative btn-secondary btn-lg\"\r\n\t href=\"https:\/\/ocean.cognisantmd.com\/intake\/patients.html?linkRef=ee11aa81-2fcd-4b92-a2aa-4fb97c05ae62#\/online-booking\" rel=\"noreferrer noopener\" target=\"_blank\" >\r\n\t\t\t\t\t\r\n\t\t\t\t\tBook an appointment at the Access Clinic \r\n\t\t\t\t\t\r\n\t\t\t\t\t \r\n\t\t\t\t<\/a>\r\n\t\t\t\n\n\n<p class=\"has-medium-font-size wp-block-paragraph\"><\/p>\n\n\n\n<p class=\"has-large-font-size wp-block-paragraph\">What is team-based care?<\/p>\n\n\n\n<p class=\"has-medium-font-size wp-block-paragraph\">Primary care providers (physicians and nurse practitioners) are supported by nurses, allied health providers and physician specialists.
